Community Champions

Specializing in vulnerable populations, TrueCare understands the impact of community-based social services and programs on health outcomes; this concept is an integral part of our model. Community Innovation Grants are intended to strengthen the social safety net that supports our most vulnerable communities. These funds are unrestricted – allowing our social service organizations and leaders the ability to use funds to flex their innovative muscles as they approach unique community needs. We invite our non-profit community-based social service providers to apply for awards to support critical services and programs. We also invite you to share this link with community leaders who are doing exceptional work with underserved communities.
